FVIP Points for Referral Sources

• This is a 26-week educational program held in a group setting, which meets once a week for 90 minutes.

• The same topic is presented Monday through Sunday of each week.

• A client referred to our program must complete an Introductory class, 26 classes and an exit interview.

• Each client is allowed to have four (4) absences, but these absences must be made up at the end of the program. Clients must attend at least 26 classes to complete the program.

• Clients will receive four (4) letters regarding their non-attendance.

• Our fee for perpetrators is $675.00 ($25.00 per class including introduction). When both partners are responsible, the fee may be split ($337.50 each, $12.50 per class).

• Victims attend for free.

• All clients referred to the program are expected to pay the fee in full, before they will receive a certificate of completion. However, we will take into account special circumstances, and negotiate payment plans.

• Fees must be paid in the form of a money order or certified check. NO CASH.

• It is strongly recommended that both partners attend the program.

• Please note victim contact information on the referral form, so that we can inform the victim of the perpetrator’s enrollment status, as required by state standards.

• Please note on the referral form other programs the client has been referred to, such as The Phoenix Center or Mental Health.

• A parent or legal guardian must accompany youth under the age of 18.

• Introductory classes are held weekly. Clients must call 467-3434 to sign up.

• Client should bring a copy of court order, referral form, and/or treatment plan when they attend the introductory class.

• Written proof of attendance will be given to client if requested.

• Referral sources may verify a client’s attendance by contacting Linda or Haley at 467-3434.

• Clients are welcome to attend the program voluntarily after completion
(free of charge).
